Georgia
Georgia does not have any state family leave. Therefore, if a Checkly employee, who’s registered in Georgia, becomes a non-birthing parent, there are no state plans available to them and Checkly pays the full parental leave.
How to apply:
- Request for the leave in Humaans → “Parental”
- Ping the People team when it’s approved in Humaans, so they can include it to the monthly PTO sync with HSA.
- HSA will not make any changes in the payroll system, so pay and benefits continue as normal.

a copy of the birth certificate, if available, naming you as the second parent, a Voluntary Acknowledgment of Parentage (Form LDSS-5171), or a Court Order of Filiation.

A copy of the child’s birth certificate, if available, or an original copy of a health care provider certification of birth.
and a second document verifying the relationship to the birth parent (for example, marriage certificate, civil union, or domestic partner document).
New York
New York has “bonding leave”. The employee can take up to 12 weeks of bonding leave in the first 12 months of the baby’s birth. They are eligible to get paid up to 67% of their weekly benefit (= base salary) up to a cap. The maximum weekly benefit for 2025 is $1,177.32.

If the employee is using any paid time off during the leave, Insperity would take the benefit deductions from the paychecks. To avoid this: after the 12 weeks, we would take the employee off of their LOA (Leave of Absence), pay them in full as if they were working full time and then they would maintain their benefits.
How to apply:
- Apply for the leave in Humaans → “Parental”
- Inform HSA (Jen, Mary and Katie, cc [email protected]) that you’re going on a leave of absence
- When the baby is born, submit the filled out PFL-1 & PFL-2 forms (find them here) and supporting documentation (see “required documentation” here). Your Employer of Record is Insperity, please list “Insperity” as you employer, not Checkly.
- Once you submit the forms, Insperity will be notified to complete their sections.
- Please email [email protected] and HSA (Jen Mary and Katie) once you’ve submitted the forms so that we can be sure to track the progress.
